Information you send us through the website
Consultation and contact forms
The forms on the Contact Us and Dr. Mike Shin pages ask for your name and email address (required), and optionally your phone number and the best time to reach you. We also record which page the form was sent from and the time it was sent.
When you submit a form, your details are saved in a database hosted by Cloudflare and an email notification containing them is sent to the practice using Cloudflare's email sending service. We use this information to reply to you and arrange a consultation.
The forms do not ask for medical information. Please do not include medical history, photos or other health details in a website form; discuss them with the practice directly.
Blog comments
If you leave a comment on a blog post, we collect your name, email address and comment. Comments are saved in the same database, held for moderation, and a notification is emailed to the practice. If a comment is approved, your name, comment and its date are shown publicly on the post. Your email address is not published.
If you tick "Save my name, email, and website in this browser for the next time I comment", we set a cookie that remembers your name and email address for future comments (see Cookies below). If you leave it unticked, no such cookie is kept.
Spam and abuse protection
Forms use Cloudflare Turnstile, a security check provided by Cloudflare, to tell people from automated spam. Turnstile loads from Cloudflare when a page with a form is opened and processes technical information about your browser and connection. To limit repeated submissions, the website temporarily stores a one-way coded value derived from your IP address and the form type; these records expire automatically after about two hours.
Information collected automatically
The website is hosted on Cloudflare. Like any web server, Cloudflare processes technical information such as your IP address, browser type, the pages requested and the time of each request in order to deliver and secure the website. The website keeps operational logs of requests and errors so we can keep it working; form contents are not written to these logs.
Third-party services on the website
- Cloudflare provides hosting, the form database, email notifications and Turnstile security checks.
- Google Maps displays our office locations on the Contact Us page. When that page is opened, the map is loaded from Google, which receives your IP address and technical browser information. Google's use of that information is governed by Google's own privacy policy.
- Google Analytics is used only if you agree to analytics cookies (see Cookies below). If you agree, Google receives information about your visit, such as pages viewed, your browser and device, and your IP address.
Cookies and similar storage
Cookies are small files a website stores in your browser. This website uses the following:
- Cookie choice (strictly necessary): when you choose Accept all, Refuse optional or Select optional, your choice is saved in your browser's local storage under the name
kings_cookie_consentso we do not ask again on every page. It is kept for up to 12 months. - Comment details (only if you ask for it): the
kings_comment_prefscookie stores your name and email address in encrypted form so the comment form is pre-filled next time. It is set only when you tick the box on the comment form and lasts up to one year. Submitting a comment with the box unticked removes it. - Security checks (strictly necessary): Cloudflare Turnstile and Cloudflare's security services may use cookies or similar technology to protect the forms and the website from abuse.
- Analytics (optional, off until you agree): if you choose Accept all, or turn on analytics under Select optional, Google Analytics is loaded and may set cookies such as
_gaand_gidto distinguish visits. Their lifetimes are set by Google.
When you first visit, analytics stays off until you choose. You can change or withdraw your choice at any time with the "Cookie settings" link in the website footer. Withdrawing consent stops analytics from loading on future pages and removes the analytics cookies this website can access. You can also delete cookies and site data in your browser settings.
How long we keep information
Form submissions and comments are kept for as long as the practice needs them to respond to you, manage its records and protect the website. To ask about information you have sent through the website, contact the practice using the details above.
Your choices
You can choose not to use the forms and contact the practice by phone instead. You can refuse optional cookies, change your cookie choice at any time, and ask the practice about the information you have sent through this website, or ask for it to be corrected or deleted, using the contact details above.
Changes to this policy
We may update this policy when the website or the services it uses change. The date at the top of this page shows when it was last updated.
